Your Birth Plan
A birth plan is a way to express your wishes for your childbirth experience before you actually go into labor. We recommend that you complete a plan well before your due date, so you can discuss what's important to you with your partner, your physician, your midwife and whomever else may be involved in supporting you and caring for you.
This form helps you think through the details of what is certain to be a memorable time for you and your family. Please understand that it is meant to be flexible and that circumstances may require a last-minute change of plans. What's most important is the safety of you and your baby.
