Rehabilitation Services at Home
Depending on your needs, you may benefit from home rehabilitation services, such as:
- Occupational therapy, which helps you improve the activities of daily life
- Physical therapy, which is focused on improving mobility, strength and coordination
- Speech-language therapy, which treats feeding, swallowing and communication challenges

Am I Eligible for Rehabilitation Services at Home?
You may be eligible for home rehabilitation services if you:
- Are facing permanent disability from an illness or injury
- Are in stable condition
- Have received a referral for therapeutic services through a home health agency

How to Access Rehabilitation Services at Home
You must have a physician referral for home health rehabilitation services and must meet qualifications of being home bound due to your illness or injury. If you have questions about receiving rehabilitation services at home you can call (804) 628-HOME to get answers to your question. We will be happy to work with your physician on obtaining an order if they deem our services necessary for your continued care.
What Are the Benefits of Rehabilitation Services at Home?
Home rehabilitation services are a great option for patients who might find it difficult to get to outpatient rehabilitation due to physical or other challenges.
If you’re being discharged from the hospital, you will also receive care coordination for transitional care as you move from the hospital to home. It’s just one way we support your seamless recovery.
