No matter your health needs, we bring specialized care programs to support our patients at home, including:
- Hospital at Home Hospital at Home, which supports patients with acute illness receiving hospital care at home
- Specialized primary care or rehabilitative services at home such as physical therapy, occupational therapy or speech needs
- Hospice care, which provides end-of-life and palliative care for patients and support to loved ones
- Infusion therapy and dialysis, which gives the training, supplies and in-person support for patients receiving infusion medication
- Virtual care and remote monitoring, which helps your care team monitor chronic or acute conditions without needing to travel to the hospital or clinic
Additional Services:
Many of our health at home services are possible because of skilled nursing care. Skilled nurses will come to your home for in-person visits for the following:
- Catheter care
- Injections
- Medication management
- Oxygen management
- Sample collection for laboratory tests
- Vital sign monitoring
- Wound care
Some treatments, including wound and ostomy care, require regular care from a doctor or nurse. VCU Health at Home services now make it possible for some patients to receive this level of care at home, which means fewer trips to the hospital or clinic and reduced stress as you recover where you're most comfortable.
Your care team will clean the wound of any debris, dress the wound and help promote the growth of healthy tissue with advanced therapies.

If you need respiratory care for a chronic condition or following illness or injury, our team of skilled experts is here for you with support, including oxygen therapy.

You can receive imaging services from the comfort of your home, including:
- Echocardiograms (ECG or EKG) to measure the electrical activity of your heart
- Ultrasounds that use sound waves to create images of internal organs or tissues
- X-rays that use radiation to look at bones, joints and other tissues
You can receive pharmacy services, including medication management and reconciliation, from home. This service makes sure you have the most accurate list of your medications and how you use them. This support means fewer errors, a lower chance of hospitalization and better health outcomes for you. With virtual medication reconciliation, you receive support from the comfort of your home, avoiding visits to multiple providers and specialists to coordinate your medications. In cases where you may need assistance to take medication (including IV medications), a skilled nurse will come to your home to assist.
We prescribe, order and deliver directly to your home. Equipment and supplies include:

Medical social work services can provide a bridge between you, your care team and other aspects of your life like school, work, transportation, housing and meals. These services help you or your loved one recover with peace of mind that these other needs are taken care of.
We provide free social work services to patients at home to help you with decision making and participation in your care. Our team can help you with services including:
- Assistance with rent or utilities
- Counseling and mental health coordination
- Help coping with a diagnosis
- Medical equipment in the home
