Your Care Team
Your VCU Health at Home providers cross specialties to ensure you have comprehensive care from the comfort of your home. By collaborating and communicating with all of your providers, including your primary care physician, we ensure you have continuity of care, wherever you are.
Your health at home care team may include:
- Bereavement coordinators, who help family members through the process of losing a loved one
- Care coordinators, who make sure all the different parts of your care, including appointments, monitoring, medication, meal deliveries and more, work together
- Doctors, across specialties, including internal medicine, emergency medicine, hospital medicine palliative care and more, who work with you to develop a care plan for your condition
- Home care nurses, who come to your home to provide skilled care like delivering medication and wound care
- Hospice nurses, who come to your home and assist with keeping you or your loved one comfortable
