VCU Health at Home

You deserve the best health care—no matter where you are. Thanks to VCU Health at Home services, specialized  care may now extend into your own home. We treat patients in Richmond and its surrounding areas throughout Central Virginia.

Depending on your condition, you may receive virtual care or in-person visits daily, have continuous health monitoring from a specialist, or simply meet with a provider for a single virtual urgent care appointment.

Care You Can Trust

VCU Health is a leader in home-based health care with more than four decades of experience caring for patients in our community. Whatever your health needs are at home, our care team customizes your treatment plan to support you and coordinates your treatment with other members of your care team. This collaboration means a streamlined and personalized experience for you and your evolving needs.

Is VCU Health at Home Right for Me?

VCU Health at home is not a one-size-fits-all approach. You may be eligible to receive care at home with access to resources at different points in your health care journey, including following a discharge from the hospital or emergency department, continuous care following an outpatient visit, or even for a one-time urgent care visit.

You may be eligible for VCU Health at home services if you:

  • Are in stable condition
  • Have a low risk of complications
  • Have complex care needs or co-occurring diseases
  • Receive daily treatments including dialysis or certain medications
