Before Your Appointment

Prepare for Your Visit

To ensure a successful visit, please bring the following items to your appointment:

• Photo ID
• Your insurance card
• Your co-pay
• Medical records such as MRI or CT reports
• A CD with images of any X-rays, MRI scans or CT scans
• Notes from your referring physician

You can also print and complete the following forms to expedite your appointment:

• Patient History Form
• Medication Reconciliation Form
