Medical records include, but are not limited to a patient's medical history, test results, office visit notes, discharge summary, and operative reports of treatments and medical services. The VCU Health Department of Health Information Management can provide you with copies of your medical records related to care at a specific facility. To request a copy of your medical record(s), please contact our Health Information Management Department at (804) 828-4423.
You can use VCU Health MyChart, our secure online patient portal for your health information. Your health information is available to view, download, and share.
Once logged in, click on "Menu" and then click "Share My Record." Learn more about sharing your MyChart record.
Download the authorization form for the facility from which you are requesting records. If you received care at the following hospitals, please visit the respective website for information on how to obtain your medical record:
