What is CMH's Pharmacy Connection?

VCU Health Community Memorial Hospital and VCU Health Community Memorial Hospital Foundation (CMHF) offer a valuable service, at no cost, to qualifying patients. CMH's Pharmacy Connection is a program that offers patients assistance in obtaining their prescription medications, eyeglasses, and medical supplies, especially if they fall into a coverage gap.

CMH's Pharmacy Connection utilizes software that includes information on over 7,000 medications. Many prescription drug manufacturers have patient assistance programs for uninsured, underinsured, and low-income patients.

How do I know if I qualify?

While there are general income guidelines (guidelines vary by each medication), many manufacturers qualify patients on a case-by-case basis.

Who do I call?

CMH's Pharmacy Connection is administered at CMH through CMH Foundation. The number to call for more information or to schedule an appointment is (434) 447-0856.

Staff members are Patient Advocates, serving as liaisons between drug manufacturers, physicians, and qualifying patients.

What do I need when I apply?

As mentioned, eligibility is based on household income, insurance coverage (or lack thereof), and availability of the prescription drug in need. Documentation needed for initial applications include:

  1. Proof of Household Income (Tax Return, Social Security Benefit Statement or current pay stubs within the last 2 months)
  2. Updated Insurance Cards (if you have insurance)
  3. Medication List (you may attach a list or list on the application)
  4. Prescribing or Referring Doctor Name/Location (please note on the application)
