Your medical records are a written history of your health. Records might include your test results, discharge summaries, notes from your doctor and reports of past care. You can use your medical records to follow treatment plans, prepare for appointments and have better conversations with your doctors and other providers.

Previous portal access
As of December 4, 2021, MyChart is VCU Health’s patient portal. The previous My VCU Health Patient Portal is now view-only. You can access the previous portal here.
Proxy access
Proxy access lets you share parts of your EHR with family members and caregivers who are 18 and older. Someone with proxy access can help you make appointments, view your medical records and message providers.
You should never share your MyChart log in information with anyone. Proxy access is the safest way to share your account.
Types of proxy access:
- Full Access: Anyone with full access can schedule appointments, send messages to providers and view a patient's clinical chart.
- Clinic Access: Anyone with clinic access can view a patient's clinical chart.
- Scheduling/Messaging Access: Anyone with scheduling/messaging access can schedule appointments for the patient and send messages to providers.
How to give proxy access:
- Log into your MyChart account
- Scroll to the “Explore More” section at the bottom of the main page
- Find the “Invite Friends and Family” option, then tap or click “Manage Access”
- Tap or click the option that says, “Invite friends or family”
- Follow the instructions that MyChart gives you, a name and email address are required
- Select the type of access you want to give
- To complete the process, MyChart will send an invitation to the email address you provided
- The recipient will need to log into their MyChart account, accept the invitation and enter your date of birth

Health information amendment requests
To request a change to your medical information, download and complete the medical record amendment form and then send it to VCU Health's Health Information Management Department by email, mail or fax. Learn more about health information amendment requests here.
Health information exchanges (HIEs)
Health information exchanges (HIEs) allow our doctors, nurses, pharmacists and staff to appropriately access and securely share your vital medical information electronically with other health care providers, hospitals and clinics. This improves the speed, quality, safety and cost of your health care.
VCU Health participates in one or more HIEs. Your health information will be shared in accordance with federal and state law, as well as our own Notice of Privacy Practices.
You have the right to opt out of HIEs. Please note that if you choose to opt out, providers will not be able to electronically obtain your medical information from HIEs, but will instead obtain it by other means, such as fax or mail.
Download the HEI opt out form here, fill it out, and send the completed form to us by mail, email or fax.
